At Home Care and Services for Seniors

GENERAL INFORMATION

There are some services available in Georgia that may make it possible for a senior citizen to stay in his or her home rather than move to a nursing home or personal care home.

COMMUNITY CARE SERVICES PROGRAM

The Community Care Services Program is available to Medicaid-eligible individuals, and provides various services to eligible senior citizens, like homemaker services, home health care services, in-home personal care, adult day rehabilitation, emergency response, respite care, and home-delivered meals (if another Community Care service is being provided). To apply for the program or to get information about the program, you should contact the Area Agency on Aging ("AAA") for the county in which you live. For assistance in contacting your local AAA, call the Georgia Division of Aging at (404) 657-5258, or click onto the Resources and Links page under At Home Care for website information on AAA.

If you apply for the Community Care Services Program, a member of a "community care assessment team" will visit you and fill out a form on your situation. You will be eligible for the program if your doctor certifies that you are medically eligible for nursing home placement and the community care assessment team determines that you will be able to live at home with help from the program. If you are eligible, the community care assessment team will assign a case manager to you.

Your case manager will arrange for the services that you need and will figure out any cost to you. The Community Care Services Program may pay the cost of the services that you receive through Medicaid or may share the cost with you. Medicare also may pay all or some of the cost of any home health care services that you receive.

Please be aware that the Community Care Services Program uses the Medicaid rules about transfers of resources, which could affect your eligibility for the program. OTHER AT-HOME SERVICES

There are a number of programs in Georgia that provide services to people who are 60 or older, like meals-on-wheels, homemakers, visiting nurses, and transportation to the doctor. Please see the Resources and Links page under At-Home Care for a detailed list of these programs.
